STEP 02

Create your Parent Portal

Emergency Contact forms must be completed before starting your Parent Portal registration.

STEP 03

Submit Health Forms before first day of school

You must submit all forms listed below using “Upload File” link provided. Please call us if you have questions.

• Current immunization records
• Health statement from your child’s doctor stating they are cleared to participate in our program
• Hearing and vision test results are required for 4 and 5 year old children.
